1 LIGHT VEHICLE INCIDENT Date: 30-Jan-2016 Time: 07:30 Driver: Abd Alkareem Abd Alrahman Location: WQ1 field (close to DS7) o Oilserv driver was traveling back to Oilserv base with CTS night-crew personnel from WQ1-042 on 3 rd Party Oilserv pick up when a parked civilian car drive in suddenly enter to main road from right side of the road. Oilserv driver tried move away and stopped after get hit by the other car. o The incident was reported to Oilserv Base and Safety Officer was dispatched to the location, and the 3 rd Party pick-up owner informed to come to location o Safety Officer observed and interview people involved in the accident and come back to Oilserv base with CTS crews o 3 rd Party pick-up owner will have internal discussion with civilian car driver and will update Oilserv

2 LIGHT VEHICLE INCIDENT o Immediate findings: Oilserv Driver Defensive Driving Training and Commentary Drive Assessment are up to date. Iraqi driving license: DD Training & CD Assessment: 12-Jan-2016 This is the first trip of Oilserv driver for today with enough rest over the last night JM Management form and Pre-Trip check were completed. The vehicle was equipped with operable IVMS unit. The seat belt was buckled up.
